LOS ANGELES -- Scott Weiland died accidentally with a mix of cocaine, alcohol and MDA, also known as ecstasy, in his system, according to a toxicology report performed on the Stone Temple Pilots and Velvet Revolver frontman. Weiland died earlier this month in a Minneapolis suburb.

Weiland had other "significant conditions" -- heart disease and asthma -- that contributed to his death on Dec. 3, according to documents from the Hennepin County Medical Examiner obtained by Mashable (below).

Weiland was found dead on his tour bus during a stop at the Medina Ballroom in Bloomington, Minnesota, where he was on tour with Scott Weiland & The Wildabouts. The medical examiner said Weiland's history of drug and alcohol abuse was also a factor.

He'd recently been dismissed from Stone Temple Pilots, whose remaining members had nothing but warm things to say about their co-founder, even months after his exit. But the habitual substance abuse had taken its toll on the professional relationship -- and he left Velvet Revolver in 2008 -- leaving him to tour with the Wildabouts.
